What C677T and A1298C actually mean

For anyone staring at a result like 'C677T heterozygous' and wondering if they should worry.

You got a result back — maybe from a test, maybe from a group member reading your raw data — and it says something like “C677T heterozygous” or “compound heterozygous.” Your heart sank a little. Let’s translate it, calmly.

The two names

MTHFR has two common variants that get nearly all the attention:

  • C677T (sometimes written rs1801133)
  • A1298C (sometimes written rs1801131)

The letters and numbers just describe the spot on the gene and the swap. “Heterozygous” means you have one copy of the variant; “homozygous” means two; “compound heterozygous” means one of each (one C677T and one A1298C).

What each result generally means

A gentle, honest guide — not a diagnosis:

  • No variant / “normal”: MTHFR is doing its job at the usual pace here. Nothing to act on.
  • One copy of C677T (heterozygous): Usually only a modest reduction in this enzyme’s activity. For most people, not a concern on its own.
  • Two copies of C677T (homozygous): A more noticeable reduction — you’ll often see roughly a two-thirds drop quoted. Worth understanding calmly; not worth fearing.
  • A1298C (one or two copies): Tends to have a gentler effect on its own than C677T.
  • Compound heterozygous (one C677T + one A1298C): Gets a lot of dramatic attention online. It can modestly lower activity, but the sky-is-falling framing you may have read is not supported the way those posts imply.

The three things the internet won’t tell you

  1. These are common. A large share of people carry one or both. “Having a variant” is closer to normal human variation than to a diagnosis.
  2. Activity, not identity. A result describes how fast one enzyme step runs — not who you are or what you’re doomed to.
  3. Context is everything. The same result means different things depending on your folate and B12 status, the genes around MTHFR, and your real-world labs. A number in isolation can’t tell you much.

What to do with your result

Honestly? Take a breath first. Then get curious rather than scared. The useful next step isn’t to memorize your MTHFR letters — it’s to see them in context: alongside the genes that work with MTHFR, and translated into everyday food and nutrient choices you can actually use.
